新生體檢專區

依據教育部《學校衛生法》第8條：「學校應建立學生健康管理制度，定期辦理學生健康檢查。」、《學校健康檢查實施辦法》第4條：「學校辦理新生入學時，應進行學生健康基本資料調查，並做成紀錄。」及《中國醫藥大學新生健康檢查實施要點》第5條：「未依規定完成健康檢查之學生，視同未完成入學程序。」故為維護學生健康，早期發現健康問題、早期治療，並預防傳染病，本校新生入學時依規定應完成體檢。

一、實施對象：113學年第一學期入學新生(含大學生、碩博班、復學生、轉學生、外籍生)

二、檢查方式：

（一）到校參加新生體檢：

體檢地點：校本部歷史建築區人文藝術中心（二樓）

體檢時間：請各系所依規劃時段準時前往，以免造成流程阻塞。

                      11：30前須完成報到程序。

(二)自行至中國附醫體檢：

體檢時間：113年09月05日至113年09月30日止，採預約方式，詳請見中醫大補檢注意事項       

體檢費用：規定期間內參加體檢者，享優惠價1100元/每人；逾期繳原價3930元。

(三)自行至衛福部評鑑合格之醫院檢查：

請自行下載中國醫藥大學新生體檢表，攜表至評鑑合格之醫院檢查。體檢費用依各醫院規定。

體檢報告須為開學前3個月內有效之報告，請於開學後二週內（9月23日前）繳交體檢報告至各校區健康中心衛保區：

  1. 校本部（水湳校區）位於關懷大樓2樓，分機1250
  2. 英才校區位於互助大樓4樓，分機1251
  3. 北港校區位於教學大樓1樓聯合辦公室-學務組護理師，分機1200

三、體檢注意事項：

1.檢查前請空腹8小時，禁食禁水。

2.當日請攜帶身份證 (非本國籍學生請攜帶護照正本+護照影本)、健保卡、檢查費用。

3.檢查當日請著輕便服裝（勿著有扣子或金屬成分裝飾的內衣褲），請勿攜帶貴重物品（例如：項鍊、戒指等）。

 

 

Information for CMU New Student’s Physical Examination

A. New students enrolled in the first semester of the 113th academic year (including undergraduates, master's and doctoral students, returning students, transfer students, and international students).

B. Examination Method:

a. Attend the on-campus health check for new students:

Location: Humanities and Arts Center (2nd Floor) in the Historic Building.

Time: Students from each department should arrive promptly following the scheduled time.

Examination Fee: NT$1,100 per person (Payment should be made on the day of examination.)

★Notice for the physical examination:

  1. Please fast (no food and drinks) for 8 hours before the examination.
  2. Please wear light clothing (do not wear underwear with buttons or metallic decorations) Bring your  ID  card  (or passport and its copy if you are an international student),  NHI card (if any) and the examination fees. Do not bring valuable belongings (ex: necklaces, rings, etc.)

  1. Students who fail to finish all examination items during the on-campus check can bring your supplementary checkup form and passport and go to 8F, Center of Health Evaluation and Promotion, CMUH (Wuquan Road & Jinxin Street intersection) to complete the rest. Fast for 8 hours is needed if you are going to have the blood test that day.

For those unable to participate in the on-campus health check, please follow the procedure below:

4.Examination location and fees:

(1)China Medical University Hospital:

 

  • Reservation 3 days ahead is needed (not including holidays and weekend). Please fill out the reservation form (download it here) and send your reservation to adm15@mail.cmu.edu.tw Deadline for reservations is 10:00 every morning. You will receive confirmation from the hospital via text message once reserved, please arrive the location based on the time arranged. If you need to change the reservation time, please email the health center again
  • Date for the examination: From September 5-30, 2024. Examination fee after September 30 will become NT 3,930.
  • Time for the examination:

Morning: 08:00-10:00, Monday-Saturday, please arrive and register before 9:30 (Dental check is only available in the mornings).

Afternoon: 13:00-16:00, Monday-Friday

  • Location: 8F, Center of Health Evaluation and Promotion (Wuquan Road & Jinxin Street intersection)

 

  1. (2)If you wish to go to other hospitals for the examination, please go to a regional hospital or higher level. The examination fee will vary according to each hospital’s regulation. Please bring the physical examination form, passport, ID card (if any), NHI card (if any) and money with you.

  • Please submit the report of the physical examination that is done within 3 months to the University’s Health Center (no later than September 23).
