​ 新生體檢專區

依據教育部《學校衛生法》第8條：「學校應建立學生健康管理制度，定期辦理學生健康檢查。」、《學校健康檢查實施辦法》第4條：「學校辦理新生入學時，應進行學生健康基本資料調查，並做成紀錄。」及《中國醫藥大學新生健康檢查實施要點》第5條：「未依規定完成健康檢查之學生，視同未完成入學程序。」故為維護學生健康，早期發現健康問題、早期治療，並預防傳染病，本校新生入學時依規定應完成體檢。未於期限內完成新生體檢並繳交報告者，將依本校相關規定辦理，請務必留意期限。

一、實施對象：114學年第2學期入學新生(包括復學生及轉學生)

二、檢查方式：

您可自行選擇以下方法(二擇一)完成新生體檢

一、至中國醫藥大學附設醫院體檢：

1.體檢日期：115年02月02日～115年03月07日
　⚠️備註：2/15–2/22（農曆春節）及2/27–2/28暫停體檢服務，請提早安排。
2.體檢時間：週一至週六 08:00–10:00
3.報到地點：中國附醫-健康醫學中心8樓
4.預約方式：請填妥以下【新生體檢預約表單】

　⚠️為維護現場服務品質與作業流程順暢，現場僅提供體檢服務，請務必提前填寫預約表單，現場恕無法臨時安排體檢，敬請配合。

5.檢查前請空腹8小時，禁食禁水。

6.請攜帶身份證 (非本國籍學生請攜帶護照正本+護照影本)、健保卡、檢查費用現金1200元直接至現場報到即可。

7.檢查當日請著輕便服裝（勿著有扣子或金屬成分裝飾的內衣褲），請勿攜帶貴重物品（例如：項鍊、戒指等）。

8.於規定期間內參加體檢者，享優惠價1200元/每人；逾期繳原價3930元。

9.健康醫學中心地址：五權路與錦新街交叉口。

10.體檢結束後，您的體檢報告會統一由附醫寄送至健康中心，屆時再個別通知領回。

二、至其他評鑑合格醫院體檢：

若您到其他評鑑合格醫院體檢，請先下載中國醫藥大學新生體檢表攜表至評鑑合格之醫院檢查。體檢費用依各醫院規定。請於開學後二週內（3月9日前）親自繳交體檢報告影本至各校區健康中心衛保區或是寄送體檢報告檔案至電子信箱。

三、因勞工體檢報告及外籍生簽證體檢報告與教育部規定新生體檢表內容相差甚多，不符合規定，故請重新做新生體檢，方可完成入學程序。

四、轉學生須完成新生體檢。  休學生暫免，復學時再完成體檢。

Guidelines for Health Examination
New Students Admitted in Spring, 2026

You may choose one of the following two methods to complete the new student health examination:

1.Checkup at China Medical University Hospital

(1) Health Examination Period: February 2, 2026 to March 7, 2026 (Deadline)
Note: Health examination services will be suspended during the Lunar New Year holiday (February 15–22) and on February 27–28. Please make arrangements in advance.

(2) Examination Hours: Monday to Saturday, 08:00–10:00 a.m.

(3) Registration Location: 8th Floor, Center of Health Evaluation and Promotion, China Medical University Hospital (At the intersection between Wuquan Road & Jinxin Street, please refer to the photos below)

(4) Reservation Requirement: Please complete the form below (by clicking the link or scanning the QR code) and make an appointment in advance (at least 1 week ahead). Walk-in or last-minute appointments are not accepted.

(5) Fasting Requirement: Please fast for 8 hours before the examination. No food or water is allowed.

(6) Required Documents & Fee: Please bring your ID card (or passport and its copy if you are an international student), NHI card (if any), and NT$1,200 in cash for the examination fee. Register directly on-site upon arrival.

(7) Dress Code & Personal Belongings: Please wear comfortable clothing (do not wear underwear with buttons or metal components). Avoid bringing valuables such as necklaces or rings.

(8) Examination Fee: Students who complete the health examination within the designated period are eligible for the discounted fee of NT$1,200 per person. The regular price of NT$3,930 will apply if the examination is completed after the deadline.

(9) After completing the health examination, your report will be sent directly by China Medical University Hospital to the Health Center on the 2nd floor of the Care Building, Shuinan Campus. You will be notified individually when your report is ready for collection.

2. Health Examination at Other Accredited Hospitals

If you choose to complete your health examination at another accredited hospital, please first download the China Medical University New Student Health Examination Form and bring it with you to the accredited hospital for the examination. Examination fees will be charged according to each hospital’s regulations.

Please submit the completed health examination report in person to the Health Center at your respective campus within two weeks after the semester begins  (by March 9) or email a scanned copy of the report to the designated email address.

3. The labor health examination and the international student visa health report do not align with the Ministry of Education’s required new student health examination form. Therefore, a new student health examination must be completed to fulfill the enrollment requirements.
