113-2學期新生體檢專區

依據教育部《學校衛生法》第8條：「學校應建立學生健康管理制度，定期辦理學生健康檢查。」、《學校健康檢查實施辦法》第4條：「學校辦理新生入學時，應進行學生健康基本資料調查，並做成紀錄。」及《中國醫藥大學新生健康檢查實施要點》第5條：「未依規定完成健康檢查之學生，視同未完成入學程序。」故為維護學生健康，早期發現健康問題、早期治療，並預防傳染病，本校新生入學時依規定應完成體檢。

一、至中國醫藥大學附設醫院體檢：

1.體檢日期：114年02月03日 至 114年03月02日截止

2.體檢時間：星期一至六早上08:00-10:00

3.報到地點：中國附醫-健康醫學中心8樓

4.請填妥以下表單，提早3-5天預約，臨時及現場預約恕無法受理

備註：表單提交後，系統會自動寄發信件至信箱，並不代表預約成功，當您收到醫院給您的簡訊，才表示預約成功。收到簡訊後可直接於預約當日至中國附醫體檢。

 

5.檢查前請空腹8小時，禁食禁水。

6.請攜帶身份證 (非本國籍學生請攜帶護照正本+護照影本)、健保卡、檢查費用現金1100元直接至現場報到即可。

7.檢查當日請著輕便服裝（勿著有扣子或金屬成分裝飾的內衣褲），請勿攜帶貴重物品（例如：項鍊、戒指等）。

8.於規定期間內參加體檢者，享優惠價1100元/每人；逾期繳原價3930元。

二、自行至其他評鑑合格醫院體檢：

若您到其他評鑑合格醫院體檢，請先下載中國醫藥大學新生體檢表，攜表至評鑑合格之醫院檢查。體檢費用依各醫院規定。並於開學後二週內（3月3日前）繳交體檢報告至各校區健康中心衛保區。

三、因勞工體檢報告及外籍生簽證體檢報告與新生體檢表內容相差甚多，不符合規定，故請重新做新生體檢，方可完成入學程序。

四、自行繳交報告，請於開學後二週內（3月3日前）繳交體檢報告至各校區健康中心衛保區：

New Students Admitted in Spring, 2025

一、Checkup at China Medical University Hospital

  1. Date: from 3rd, February to 2nd, March, the checkup fee would be NT$1,100. It will become NT$3,930 when the contract ends after 2nd, March.
  2. Time: 8:00-10:00, Monday- Saturday
  3. Location: 8F, Center of Health Evaluation and Promotion, China Medical University Hospital (At the intersection between Wuquan Road & Jinxin Street, please refer to the photos below)
  4. Please make the reservation by filling out the form 3-5 days ahead, on-site reservation is not accepted.Fill out the reservation form via Link: here or QR code

P.S. After submitting the reservation form, you will receive an email of completion, which does NOT mean it is successfully reserved. It is until you receive the text message from CMUH that it is reserved. You may go to CMUH for the checkup as reserved after receiving the message.

 

     5. Please fast (no food and drinks) for 8 hours before the examination.

     6. Bring your ID card (or passport and its copy if you are an international student), NHI card (if any) and the examination

       fees.

    7. Wear light clothing (do not wear underwear with buttons or metallic decorations). Do not bring valuable belongings (ex:     necklaces, rings, etc.)

二、Checkup at other hospitals

You may download the health checkup form here: https://ppt.cc/fe0KVx and bring it to any of the hospitals below. Please note that either the Health Center or the Office of Global Affairs are NOT aware of the medical examination fees charged by other hospitals. We kindly suggest that you inquire directly with the hospital of your choice and follow their specific guidelines.

三、 Please note you are still required to complete the “New Student Health Examination” even you have already done the health checkup for VISA outside Taiwan.

四、 Please submit your Health Examination to the Health Center before 3rd, March.
